Case Management Services
Members are provided with an array of supportive service interventions, advocacy, referrals, information, and assistance with benefits and entitlements. The case management division creates an individualized plan that meet the member’s needs and links them to appropriate services in the community.
GRIOT provides holistic coordinated care to LGBT older people and the people who support them. If you are an LGBT person age 60 or over (50 or over for those living with HIV/AIDS), we can help you. If you are a friend, family member or partner—even if you are not LGBT or over 60—who has been caring for an LGBT older person, GRIOT can help you as well.

How Do I Get Help from GRIOT?
Case management service is located at our office located at 25 Flatbush Av., Brooklyn.
Case Manager is available by appointment every Wednesday from 2pm- 4pm.
